Assignment of Dividends

any time during the calendar year, but must be received prior to the deadline date of December 15th. Please remember to include your certificate holder number. An acknowledgement will be sent confirming your request. If you do not receive confirmation within 3 weeks of making a request, please contact our office. This confirmation should be retained until you receive your refund. Notice of dividends available and the approximate percentage of premium available for refund will be published in the November issue of the ABA Journal.

Instructions for new applicants can be found on the second page of the application and apply to dividends, if any, during the first policy year only.
